Choose an Option Below to Obtain a Copy of Your Medical Records

At Mid Coast Health System, our Health Information Department is dedicated to maintaining a high level of privacy and confidentiality with all patient records. We keep all health information private and secure in accordance with federal and state regulations.

 

  • Online Form: Complete the “Online Patient Access Form” by clicking the link associated with the hospital. Once you have printed and completed the form, send via mail, fax, or email at one of our locations listed below.

  • MyCareCorner: Request a copy of your records through MyCareCorner Patient Portal. 
  •  
  • Phone: Call one of our office locations at one of the phone numbers listed below.
  •  
  • Not the Patient: If you are not the patient but would like to request medical records of a patient, download the Authorization for Use and Disclosure of Health Information form and mail, fax, or email it to the Mid Coast Health System facility where services were rendered. The form must be filled out by the patient or the patient’s guardian/legal representative.
